Long-term Conditions

If you have any of the following long-term conditions, we will contact you for an annual review in your birthday month or shortly thereafter. This review will be scheduled to take place either here in practice or in a Community Treatment and Care Centre, which are located throughout the city.  You may also need further checks outwith this schedule. If you feel you are overdue your check-up,please contact reception to let us know.

The conditions are:

  • Asthma and COPD
  • Diabetes
  • Impaired Glucose Tolerance
  • Coronary Heart Disease
  • Hypertension
  • Stroke
  • Peripheral Vascular Disease
  • Chronic Kidney Disease
  • Hypothyroidism

Antenatal & Baby clinics

To register a new pregnancy please visit Badger Notes. You do not need to inform the surgery of a new pregnancy. For any appointments queries or for follow up appointments please contact the midwives appointment office on 01382 740204.Child developmental screening appointments are sent to you by Tayside Health Board. If you feel your child’s appointment is overdue, please contact the Health Visitors 01382 740445. Childood immunisations are organised directly by the Child Health department of NHS Tayside (01382 432111), not by the Practice.

Sources of Support

A Social Prescribing Link Worker will be offering appointments on Thursday afternoon at Ancrum One. They can offer help if you have -Money worries, Housing issues, Problems with benefits, Lack of support, Feelings of isolation or loneliness. Details can be found here. You do not require a GP referrral for this service. Our admin can help with any queries you may have.

Patient Assessment and Liaison Mental Health Service (PALMS)

A mental health support worker is available on 1 day per week. If you are not currently being seen by a mental health professional PALMS will be able to assess your needs and help you access further support. Please call for more details.

Travel Vaccinations

If you are planning any trip overseas, it is important that you have the correct Travel Health Advice. NHS Scotland provides the latest advice on the Fit for Travel website.

If you need individual advice including vaccination or malaria prevention, please use the NHS Vaccination Service (tay.travelvaccines@nhs.scot/ 01382 423108).

GP Practices in Tayside no longer provide travel vaccinations or anti-malaria tablet prescriptions.

 

Non-NHS Services

We are an NHS General Practice but certain services we provide are not covered by the NHS and you will be asked to pay a fee. This includes but is not limited to –

  • Pre-employment medicals
  • Private medical examinations
  • Insurance claim forms
  • Holiday cancellation forms
  • Private medical certificates
  • Fitness to drive medicals

Any forms for insurance purposes, income protection, medicals both (employment and fit to travel) etc should be handed into reception and they will be passed on to the appropriate doctor for completion. Please help the doctor by completing the appropriate details, i.e. name, address, date of absence/illness.

These forms can take up to 28 days to complete but can take longer and there will be a charge as recommended by the British Medical Association, to cover costs.

All Non NHS/private service requests will only be processed once payment has been received. 

Unfortunately the practice is unable to accept cheques or card payments. We can accept cash payments in the practice and BACS payment before any treatment or certificates/letters are issued.  Please contact the practice for details.
