We offer comprehensive care and monitoring for
a range of conditions.
If you have any of the following Long Term Conditions we will try to see you for
an annual review in your birthday month or shortly thereafter. You may also need
further checks outwith this schedule. If you feel you are overdue your check-up
please contact reception to let us know.
The conditions are Diabetes, Impaired Glucose Tolerance, Coronary Heart
Disease, Hypertension, Stroke, Peripheral Vascular Disease, Hypertension,
Chronic Kidney Disease, Hypothyroidism, Asthma and COPD.
Antenatal & Baby clinics
The Community Midwife visits every Tuesday. Please make an appointment
directly with the Community Midwifery team 01382 632866 for initial
appointments and 01382 740204 for follow up appointments.
Child developmental screening appointments are sent to you by Tayside
Health Board. If you feel your child’s appointment is overdue, please
contact the Health Visitors 01382 740445. Childood immunisations are
organised directly by the Child Health department of NHS Tayside (01382
432111), not by the Practice.

If you are planning any trip overseas, it is important that you have the
correct Travel Health Advice. NHS Scotland provides the latest advice on
the Fit for Travel website.
If you need individual advice including vaccination or malaria prevention,
please ask for a travel form from reception. Alternatively you can download it
from here. Return the completed from (one for each traveller) to reception
and the Practice will contact you to arrange an appointment. Please note,
you will have to pay the pharmacist for all malaria prevention medication and
also some vaccinations as not all are provided by the NHS.
