Confidentiality
We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and our procedures are compliant with the General Data Protection Regulation and Data Protection Act 2018. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team. Usually your notes would be disclosed in an anonymised form. However, there are times when the record cannot be anonymised, either because it would be impractical to do so or because of the nature of the request. We understand, however, that sometimes things go wrong. If you are dissatisfied with something we have done, or have not done, please tell us in person, by phone or in writing and we will do our best to put things right. We are an NHS General Practice but certain services we provide are not covered by the NHS and you will be asked to pay a fee. This includes - ● Pre-employment medicals ● Private medical examinations ● Insurance claim forms ● Holiday cancellation forms ● Some holiday vaccinations ● Private medical certificates ● LGV/Taxi Medicals ● Fitness to drive medicals You will be informed in advance if there is a charge for a service.
